In this forgotten letter, largely preserved in medieval translations into Arabic and Latin, the great medical world of antiquity examines the relationship between conscious and unconscious movements. Looks at the structure of the tongue and esophagus and asks why mental concepts can have physical effects on the body. Some of his questions still bother modern scholars, although they will not accept most of his answers.

Explains the introduction and explained the extensive medical background of non-medical specialists, discussing the place of this study and anatomy in Medieval medicine down to Leonardo da Vinci. In addition to being the first English translation of this important work, this is also the first comparative study of medieval translations of the same old text, and is based on new editions and arrangements of all three.

The commentary pays particular attention to the linguistic elements involved in the making of these translations.
