Borderline personality disorder patients are reckless, unstable and devastated, hurting themselves and those around them, including those seeking help. This has led to widespread reluctance to treat them and pessimism about treatment.

In the authors' experience, this pessimism is not justified, because for many patients, relatively short intervention can be effective in terms of cost and benefit as well as human terms. The interventions described here have been used to treat outpatients for 15 years.

The results suggest that treatments can achieve clinically significant changes in the course of 24-24 sessions, in a large proportion of patients. While the Committee against Torture shares some ideas and approaches with other approaches, it offers many new features and is uniquely integrated at the theoretical and practical level.

Restoring patients' problems early can help contain destruction and create a work alliance. Also, the use of rework to teach self-reflection and avoid collusive responses from the therapist, throughout treatment, represents a powerful new technique.

The book presents a critical assessment of current ideas and practices and contrasts with the ways in which the CAT moves the patient's own resources. The authors argue that CAT must have a place in any service that seeks to help these difficult patients.
